Perhaps he is reading her signals, which is something he could not do at first. Spaghetti breath is not a reason he would ever think to excuse himself from trying to kiss her back. On the contrary, Castiel has from time to time has made it clear her cooking has made him happy. Some tongue action wouldn't bother him either.
