In all honesty, Dean isn't certain that he shouldn't cut this fishing trip short, but he's spoken to Sammy a few times and the kid seems to be happy enough. It helps ease Dean's mind that Castiel is there to check on Sammy and report back to Dean.
He's got a fishing line in the water, a little music from an old am/fm radio that has a station playing classic rock, and a beer. His best friend should be showing up any moment, assuming nothing is going wrong at the bunker, and Dean is choosing to believe it's not. After all, they'd call him if there was a need.

As he does show up, the ends of his trench coat flap behind him and give some indication of his arrival. The classic rock is so reassuringly Dean that it's first the thing he finds himself comforted by today. Honestly, it's up to Dean if something can be interpreted as wrong. He's here to report what he has seen. He's not the best at announcing himself either so he will get right to it.
"Sorry to keep you waiting. Your brother- Sam seems more fond of ice cream and red confectionery. I find it strange. Other than that, his attitude seems to be in the same spirits. They spend almost about as much time together as... you do with Sam."

When Castiel is late, Dean worries. His first thought is to call Sam, but he doesn't want Sam to know that Castiel is reporting to him. so he gives Castiel a bit of time, focuses on his fishing and drinks another beer. He visibly relaxes when Cas shows up, even if he doesn't glance back at the flutter of the trench coat. "Forget how to tell time, Cas?" Dean asks in greeting.
He looks up when Castiel does speak, brows raised a bit, eyes squinted at Cas. "Red confectionery?" Explain, Cas. He tilts his head a little at the last bit then frowns. "He's not spending time alone? Or exploring the place?" That's surprising to him. In all honesty, he's got very little idea of what Sammy does when he and John are on a hunt. He assumed that Sammy did homework and read, watched TV, but here he doesn't have any responsibilities like homework so he'd assumed that Sammy's curious nature would lead him to explore; he didn't actually believe that his kid brother would be comfortable enough to spend large amounts of time with Ruby. After all, he's shy and awkward around girls back home.
